The situation
Governance has to be built while delivery continues, because pausing a live programme to fix how it is run is rarely an option anyone will authorise.
You cannot stop the programme to install the governance. So you install it around the programme.
How we would run it
Start with the decision queue rather than the reporting. Establish who can change scope. Add stage gates at the next natural boundary rather than retrofitting them. Reporting comes last, because reporting without decision rights just produces better-formatted bad news.
What we would hand over
A working cadence, a named owner, and a portfolio view your leadership team maintains themselves.
